What is your operating system? Windows 7 SP1

Is it a 64-bit or 32-bit operating system? 64-bit

What display driver version do you have installed? 9.18.10.3257

What type of connection are you using? (HDMI, VGA, DP, DVI?) VGA, HDMI and DP, but tried others as well (see Re: HDMI port or software problem?) for details

What happens that results in the blackscreen failure? When the computer is cold-booted, i.e., shut down and rebooted, the display connected to HDMI goes black and does not get recognized anymore. Only unplugging all displays and rebooting solves the problem. Only the HDMI port is affected, the others work fine (for details see Re: HDMI port or software problem?)
